Fill in and print the following form to registrate your adopted child in the Embassy of the Russian Federation in New Zealand

The Registration Form 

It can be easily done by using courier post -  please enclose a bank cheque (50 NZD for each form), Russian passport, birth certificate, certificate of adoption, certified copies of your passports as adoptive parents, 2 passport-sized photos of the child  and a pre-paid courier envelope with your address
